Second opinions are commonly sought for treatment you are receiving, surgeries, pathology, medical tests, medications. Whatever the reason for the second opinion, properly organized medical records will facilitate the process. We’ll also supply the name of a medical provider you can contact to assist you with the second opinion. The provider will charge a fee to provide this service for you.

  • Family Medical Leave Act Claim – If you’re submitting a request for FMLA leave or have been denied a FMLA claim for an absence, well organized medical records can be helpful in resolving these situations. UNIVAL nurses will organize your medical records to support your FMLA claim.
