Getting your medical data into your personal health profile is easy – just send your records to UNIVAL and we do the rest:

	Your Google Health profile with UNIVAL:	After you pay for your order via PayPal, you will then be directed to your Google Health account. Select the profile you need to share with UNIVAL, and link your profile with UNIVAL. Your order is now complete.
	Your fax cover sheet and medical release form:	After you've placed your order, you will receive an email with a individualized fax cover sheet and a medical release form that you can use to obtain your medical records. You will see a bar code on the top of the fax cover sheet - this is your control code and it is how UNIVAL will identify your order. Note: The release form is provided as a convenience to you, but your provider may require you to complete a form specific to that office.
	Your data to import to your personal health profile:	Either you or your provider's office can fax your records to UNIVAL, including the fax cover sheet. If you would like, you can black out any name or contact information on your records, since your order is tracked by the control code on the fax cover sheet. UNIVAL's nurses will then begin the process of reviewing your records. Your records will be reviewed within 24 hrs., and you will be notified when they are complete via email. You can then login to your Google Health account and view your new data in your profile.
Payment for yourHealth is handled through PayPal. You do not have to have a PayPal account to use this service - you can pay with any credit card. For more information about how PayPal works, please visit their help center. We protect patient privacy through our stringent data security procedures and our privacy policy. Google also protects its user's privacy. Google will never sell your data. You are in control, you choose what want to store with Google and who you want to view your information.
