After you have placed your order for either our no-cost service or record conversion options, you will be directed to your HealthVault account. Select the profile you need to share with UNIVAL, and link your profile with UNIVAL. Your order is now complete.

After you've shared your account with UNIVAL, you will receive an email containing a link to UNIVAL's online form to complete your personal information, such as contact information and insurance information. Once you've submitted the form your information is immediately saved in your HealthVault account. You can return to this form at any time to update this information.

If you've chosen either our fax service or our record conversion service, the email you receive after placing your order will also include an individualized fax cover sheet and a medical release form that can be used to obtain your medical records.

Note: The release form is provided as a convenience to you, but your provider may require you to complete a form specific to that office.

Direct Import: Either you or your provider's office can fax your records to UNIVAL. UNIVAL will then organize your records, and upload them directly to your HealthVault account.

Record Conversion: Either you or your provider's office can fax your records to UNIVAL. UNIVAL's nurses will then begin the process of reviewing your records and converting your medical information.

Record Conversion with Subscription Option*: If you've selected record conversion with the subscription option simply fax your medical record release form to UNIVAL, and we will obtain your records from your primary care physician and begin the conversion process.

Your records will be completed within 24 hrs., and you will be notified of the completion via email. You can then login to your HealthVault account and view your new data in your profile.

If you've selected the subscription option simply fax any updated records to UNIVAL throughout the year and your records will be processed with 24 hrs. of receipt.

* UNIVAL will retrieve your records from one provider for the initial record conversion only. You will need to send additional medical information to UNIVAL for subsequent updates.

Payment for yourHealth is handled through PayPal. You do not have to have a PayPal account to use this service - you can pay with any credit card. For more information about how PayPal works, please visit their help center.

We protect patient privacy through our stringent data security procedures and our privacy policy. Microsoft HealthVault also protects its user's privacy. HealthVault will never sell your data. You are in control, you choose what want to store with HealthVault and who you want to view your information. HealthVault will not share your information with any third party, unless you tell them to do so.
