There is an initial set-up fee and then a small annual fee to maintain and store your personal history information. As you receive additional care, you have the ability to update your history information in our system. Make sure you always have the most up-to-date, accurate information on hand!

To protect your personal health information, at this time you are able to order yourMobile for yourself but not for any additional profiles associated with your account. In the future, as we continually refine and improve our services, we will be adding different features while maintaining the security of your and your family's personal health information at all times.

	The yourMobile history form online:	Once you've placed an order, you will be provided with a link to the history form. Complete the form online and submit it to yourHealth.
	Your personal medical history to your provider:	For your next appointment, access the yourMobile mobile web interface via your cellphone or PDA at http://yourhealth.unival-med.com/yourmobile/. Login to your account using your cell phone number and your PIN. Enter the fax number of your provider's office and click "Fax". Your personal medical history is then faxed directly to your provider's office.
	Your personal medical history at any time:	When your personal medical history changes, login to your account at our website and visit your account overview page. From there you will have access to your history form. Click the "Update" link to go to your form and update the necessary information.
