IP Address and Cookies

We may use your IP address to help diagnose problems with our server and to administer our Web site. We use session cookies to keep track of your password and to personalize your website experience. Session cookies are removed at the end of your online session.

Information Provided at Registration

UNIVAL, Inc. will protect the privacy of all of our registered users as described in this Policy. The UNIVAL registration form requires users to give us contact information (name and e-mail address), and demographic information (ZIP code and date of birth).

We use contact information from the registration form for internal purposes, to send the user news, information about new product features, services or updates, information about our company and, potentially, communication about one or more health conditions. 

Demographic and personal health information is used to tailor the visitor's experience at our site, showing them content they might be interested in, and displaying the content according to their preferences. Your account information is password-protected to prevent access by other users.

UNIVAL may contract with other companies and individuals to perform certain functions on our behalf. These companies and individuals have access to registration information as needed to perform their functions for UNIVAL. UNIVAL will ensure that any third party with which UNIVAL shares a user’s information is under a contractual obligation to use the information solely for the purposes for which the information was disclosed, however, such third parties are independent third parties over which UNIVAL exercises no control.  UNIVAL is not responsible for, and will not be liable for, their conduct, actions, omissions, or information handling or dissemination practices.  You acknowledge that your information, including individually identifiable health information, may be transmitted to and from UNIVAL through your online personal health profile and that such information may be hosted or backed-up on your personal health profile.

Except as described in this Policy, UNIVAL, Inc., will not share with third parties any individually identifiable personal information, e.g., your e-mail address, name, etc., that you supply during the registration process; this includes health insurers. You can be assured that our content providers will not have access to your personal information. UNIVAL may, however, provide your personal health information and that of other registered users in aggregate or de-identified form to third parties, but not in a way that identifies you. This means that, except as described in this Policy, we will never provide anyone's individual health records to any third party, although we may provide third parties with information about the prevalence of certain health conditions in the overall group of UNIVAL subscribers, and similar aggregate information.

UNIVAL may disclose information about you if UNIVAL believes such disclosure is reasonably necessary: (i) to comply with the law or legal process; (ii) to protect or defend the rights or property of UNIVAL or others; (iii) to enforce the UNIVAL Terms and Conditions or this Privacy Policy; and (iv) to respond to claims that the content(s) of a communication violates the rights of another.

UNIVAL, Inc. does not knowingly collect personal information from any person under the age of 13. We are committed to protecting the privacy of children, and this website is not designed or intended for children under the age of 13.

All personal, identifiable information is retained by UNIVAL, Inc. for a period of 5 years/indefinitely unless a member requests that the information be deleted. Medical Information

UNIVAL receives your medical data and processes it according to the service that you have chosen:

  • yourRecords - UNIVAL transmits your medical data to the personal health record of your choice. UNIVAL will retain digital copies of your medical records for 90 days in case of questions or disputes.
  • yourMobile - UNIVAL stores your medical history data on our servers allowing easy access from your mobile phone or PDA.
  • yourReview - UNIVAL organizes your medical records and returns them to you. UNIVAL will retain digital copies of your medical records for 90 days in case of questions or disputes.

Security

This site has security measures in place to protect the loss, misuse, and alteration of the information under our control. Information provided to yourHealth is password-protected, encrypted and protected by means of a firewall.

For yourMobile, you are also required to provide a cell phone number and choose a PIN to use as login information for the mobile web interface where you can fax your information. UNIVAL maintains the security of your personal health information by allowing only the account holder to have access to their own history data, and not being able to place orders for different profiles. As we refine this service, we will be expanding this capability while still focusing on maintaining strict security of your personal information.

Do not share either your yourHealth login information or your yourMobile PIN with anyone that you do not want to have complete access to your personal health information.

HIPAA

UNIVAL, Inc. is in compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) to the extent that law is applicable to UNIVAL as a non-covered entity.

Notification of Changes

If UNIVAL makes any material changes in our privacy policy such that it may affect any individual users, you will be notified if it affects you.
